"happening now" starts right now. jon: a fox news alert. breaking news now on the deadly terror attack on the u.s. consulate in libya. fox news sources telling us cia officers on the ground in benghazi during the attack sent requests for help but were denied. national security correspondent jennifer griffin is live at the pentagon with more on this. jennifer. >> reporter: hi, jon. well sources who were on the ground in benghazi during the attack tell fox news that in fact there were three separate requests to aid not only the attack that was occurring at the consulate and to try to help with that attack but also requests for outside help, outside military help to help once there were gunfire and firefights at the cia annex. those requests, we're told, came from higher headquarters. they were told to stand down. as you know, glen doherty and tyrone woods, the former navy seals were over at the cia annex at about 9:40 p.m. when the first shots were heard at the consulate where the ambassador and his team were taking fire. i'm told that they radioed to their higher headquart

. also pulitzer prize winning historian jon meacham. we have three weeks to go until election day. and tonight, obviously, is the big debate. it's going to be a town hall-style debate. and some new polls are out. let's take a look at, again, we've got a shift possibly in the electoral map. this morning, a new "usa today" gallup poll shows growing momentum for mitt romney in the key battleground states, including ohio, florida, and virginia. the poll has romney pulling ahead by four points among likely voters. 50% to 46%. this is obviously a big shift. it's only one poll. but perhaps the bigger take away from this poll and some others is how the candidates are ranking among likely woman voters in those battleground states. we've heard an awful lot about the gender gap since the primary season, but among likely female voters in swing states, mitt romney's pulling almost even with the president. 48% to 49%, among male voters in the swing states, romney has an eight-point lead over the president. mark, let's start with you. you're reporting right now that senior democratic officials a

pharmacy, but what the f.d.a. told us today about that pharmacy came as quite a surprise. here's dr. jon lapook. >> reporter: the food and drug administration tested 50 vials suspected to be a source of contamination in the meningitis outbreak. they found all 50 were contaminated by bacteria or fungus. the f.d.a. says internal n.e.c.c. documents show the company tested a vial from the same lot in august and labeled it sterile. the inspection also focused on the clean room at n.e.c.c. a clean room like this one is where ingredients are mixed to make sterile medication. the f.d.a. discovered n.e.c.c.'s own monitoring repeatedly found mold and bacteria growing on walls and surfaces. equipment used to sterilize medicine had "greenish-yellow discoloration" and "air ducts were dirty." according to the f.d.a., there's no evidence n.e.c.c. did anything to fix the problems. the n.e.c.c. responded today saying "we will review this report and will continue our cooperation." >> pelley: jon, thanks very much. we'll look at a key race in the battle for control of the senate. and we have the first vide

the region and the power began going out in new york city, back up generators at one hospital failed. dr. jon lapook was there as personnel raced against time to save the sick. >> reporter: the dark halls of nyu medical center led to halls submerged under 12 feet of water. >> looks like a swimming pool. >> reporter: dr. robert grossman is the ceo of the hospital. >> we had failure of our primary power, secondary power, back up systems failed within 30 minutes. so we went from functioning perfectly to being completely out of power. >> reporter: it quickly became a race against time to evacuate 300 patients including 20 newborns from the neonatal intensive care unit. when i arrived shortly before midnight, ambulances were lined up waiting to transport patients to nearby hospitals. the doctor demonstrated how hospital personnel used plastic sleds to transport patients down dark narrow staircases. >> one, make sure that they got off of the floor. the other was who was the sickest? who could go down first. >> reporter: 31-year-old christine was in the intensive care unit with complications followi
